 how you could visit our office in an affordable manner.

You can also check with your insurance company about your Out-of-Network benefits.
By calling the number on your Insurance card, you can determine what your benefits would be
if you chose to seek an opinion from Dr. Rockowitz.
We will do our best to work with those patients that
wish to seek an opinion or the services of Dr. Rockowitz.

If you are seeing us for an Industrial Injury,
your Insurance Carrier will pay for all visits as long as your Claim remains open.

If your visit is for an Independent Medical Exam, you incur no charges for that visit.

If you are a Self-Pay patient, you may check with us about
charges, payment plans, and cash discounts.
